USAFP Adjunct Membership Application

I hereby request “USAFP Lifetime Adjunct Membership” in the Uniformed Services Academy of Family Physicians. Adjunct membership provides you with attendance at the USAFP Annual Meeting at the member rate and receipt of the quarterly Uniformed Family Physician magazine. Lifetime membership dues are $100.
